How to become a new patient at Kingston Creekside Dental:

 

New patient or Dental Emergency:

Call us to book the first available appointment.

For dental emergencies we always try to see you the same day or within 24 hours, to evaluate the problem, get you out of pain, and schedule the necessary treatment for the problem.

Please arrive 15 minutes early to your appointment to fill out your Medical and Dental History Forms and to give us your insurance information (if pertinent).

You can speed up the process by downloading the forms and filling them out prior to the appointment.

Please bring with you:
  1. Government issued I.D., Driver’s License, or OHIP card for prescriptions as I.D. is now required by law.
  2. List of all medications that you are taking regularly. Feel free to have your doctor’s office or pharmacy email or fax it to us.
  3. Dental insurance card if you have coverage.Important Note: If patients do not show up for their new patient exams, we will not be able to see them in the future. This doesn’t apply to cancelling and rescheduling an appointment in the 48 hour notice period.
